One fully completed entry form below must accompany each video entry. If working as a team, all participants’ names must be included on the entry form. Only the names of those participants listed will be eligible for any prizes awarded. The lead producer will be the main point of contact and will be responsible for communications. 
All entries must be received by 11:59pm PST on October 16th, 2022

By entering this contest, each Film Maker agrees to a grant UnitedByEarth.org team members and partners a perpetual, royalty-free, nonexclusive and irrevocable license to use, promote, distribute, publish through any medium, perform and exhibit your video without further compensation or notification.*
